We advise the patients request refills within seven to ten days before they are out of medication. Consequently, the office requires 48 hours to respond to all refill requests. The exception to this is if it's an emergency. The patients have two options for requesting refills. The patient may call the pharmacy and have the pharmacy fax our office the refill request or the patient can fill out the form below with all required information and call the pharmacy at least 48 hours after the request was made to see if your medication is ready to be picked up.
